Please attach the following documents in the FormSG submission. |

1) Child’s Birth Certificate / Entry Permit for SPR |

2) Both parents’ NRIC |
|
3) Child’s immunisation record retrieved from the National Immunisation Registry website (www.nir.hpb.gov.sg) via your Singpass. |
